Privacy policy

This covers drhormonemd.com, the website, and the booking form on it. It does not cover the patient portal, where your medical records live once you are a patient. That system has its own terms, and your records there are protected health information handled under the rules that apply to a medical practice.

The booking form asks for your first and last name, email address, phone number, gender, date of birth and state. We ask for gender and date of birth because hormone care depends on both, and for your state because we can only treat patients where we are licensed.

We also record where you came to us from, such as a search or an advertisement, and the time you submitted the form.

If you go on to choose an appointment time, the calendar is provided by Cal.com. Your name and email are passed to it so you do not have to type them again, and Cal.com holds the appointment itself.

We use it to contact you about the consultation you asked for, and to tell you whether we can treat you where you live. Someone from our team reads it. We do not sell it, and we do not share it with advertisers.

If you are in a state we do not serve yet, we keep your details so we can let you know when that changes. Ask us at info@drhormonemd.com and we will delete them.

Form submissions are stored in a private spreadsheet that only our team can open. Appointments are held by Cal.com. Payments, if you go on to become a patient, are handled by Stripe, which takes your card details directly; we never see or store a card number.

We measure how the site is used with PostHog: pages viewed, buttons clicked, and whether a visit ended in a booking. This is how we tell whether the site is doing its job.

Your name, email address, phone number, date of birth and gender are never sent to our analytics. A booking is recorded against a random identifier, not against you. Web addresses are stripped of anything beyond the campaign tags before they are recorded.

Analytics is served through our own domain rather than a third-party one, which means it is not shared with an advertising network.

The site sets no advertising cookies. Analytics stores a random identifier in your browser so repeat visits are counted as one person rather than several. Clearing your browser data removes it.

Write to info@drhormonemd.com and we will tell you what we hold about you, correct it, or delete it. If you are a patient, medical records in the portal are kept for as long as the law requires a practice to keep them, and that is separate from anything described here.

This site is not for anyone under 18, and the booking form does not accept a date of birth under 18.
